Klinische Bewährung von Kronen und Brücken aus einer Lithium-Disilikat-Glaskeramik nach 60-monatiger Beobachtungsdauer

The purpose of this study was to evaluate the clinical performance of a lithium-disilicate glass-ceramic (IPS Empress 2, Ivoclar-Vivadent) for crowns and bridges. From December 1997 to June 1999, a total of 184 Empress 2- restorations (138 crowns and 46 FPDs) have been placed in 72 patients. For placement either a dual-curing resin (Syntac/Variolink II, IV) or a conventional cement (Ketac Cem Maxicap, 3M Espe, or Pro Tec Cem, Ivoclar-Vivadent) was used. The observation period was 60 months. The parameters plaque index PI (Silness and Löe), gingival index GI (Löe and Silness), probing depths, papillary bleeding index PBI (Saxer and Mühlemann), sensitivity, appearence of pain, surface texture, wear, antagonists, occlusion and esthetic were examined. The failures were evaluated using the Kaplan-Meier-method. Furthermore, influence of cementation method and loss of retention were noted. The study showed that Empress 2 is suitable for crowns in the anterior and posterior region. Using the material for FPDs, especially outside manufacturer’s guidelines very high fracture rates resulted. As a good alternative the modern CAD/CAM generated zirconium oxide ceramics can be used. Another problem was a relative high rate of chipping of the veneering ceramic which can be explained by bonding problems between the framework and the veneering material. To avoid that, a very precise manufacture is necessary. A clear influence of the cementation method could not be found. It has to be emphasized that the choice of cementation mode followed certain guidelines and did not take place randomized.
