Recurrence rates after inguinal hernia surgery are higher than expected in many studies which concentrate on special techniques of herniorrhaphy. A major problem is the big number of patients suffering from chronical pain after groin hernia surgery. This study examines four techniques of inguinal herniorrhaphy with a prosthetic mesh. Therefore we compared the TIPP technique, the Lichtenstein hernia repair, the Stoppa technique and the Wantz technique in 195 hernia repairs. We concentrated our attention on postoperative pain, disorders and the recurrence rate. Most of the followup-examinations were made after 2 years. 84% of all patients were examined. We found a recurrence rate of 13,4%. Significant risks of getting a recurrent hernia was a higher age and a bad ASA classification. TIPP herniorrhaphy got significant better results than the other techniques. 33% of all interviewed patients suffered from postoperative pain. No significant facts of risk were found. 77,6% of all patients suffered from postoperative disorders. Significant facts of risk were a high number of reoperation, a big groin hernia, adipositas and a low age.
