Optimierung der prophylaktischen Wirkung von Phospholipiden auf die Adhäsion von freien Kolonkarzinomzellen

dc:descriptionA secondary peritoneal carcinosis based for instance on colorectal cancer, is a serious diagnosis with very poor prognosis. Therapeutic options are limited, and even radical operations are mostly not curative but palliative by nature. Therefore, the goal in every treatment of a cancer in the peritoneal cavity should be the prevention of a sowing of free cancer cells or to stop their growing. Every cancer cell needs adhesion for growing. A prevention of adhesion would prevent the surface of the peritoneal cavity form metastatic settlement. This is necessary even at early cancer states because free cancer cells are found in any staging of colorectal cancers. Many groups could demonstrate the efficacy of phospholipids in preventing adhesion of intraperitoneal structures. Therefore, it seemed to be meaningful to use phospholipids for prophylaxis of peritoneal carcinosis because mechanisms of adherence of tumor cells and peritoneal epithelial cells are similar. We could demonstrate the protective effect of phospholipids in animal models and biological test tubes using different adhesion molecules. My work aimed at optimizing this effect to improve the therapeutic outcome. Female BD-IX rats were treated with different concentration of tumor cells and phospholipids, respectively or NaCl as control. Several parameters such as the peritoneal cancer index, cancer volume and surface were used to assess the degree of cancer infiltration in the rat peritoneal cavity. The results demonstrate a highly significant superiority of the highest concentration of phospholipids used (450 mg/kg) in preventing tumor cell adherence to the peritoneal suface. Even in injured areas of the peritoneal surface, in which free tumor cell adherence is know to occur very likely, high dosed phospholipids effectivly prevented metastatic settlement. So, phospholipids seem to be a promising therapeutic tool for the prevention of secondary peritoneal carcinosis in patients with gastro-intestinal cancer. In addtion to their therapeutic efficacy, no harmful effects of phospholipids could be detected in other studies. Probably due to their native chemical structure they neither cause allergic reaction nor impaired wound healing. Future work will have to adress the effects of phospholipids on integrin processes in cancer cells, and finally include therapeutic tests in humans.
