Änderung der Nierenfunktion bei Patienten mit primär maligner Hypertonie und sekundär maligner Nephrosklerose im Vergleich zu Patienten mit Glomerulonephritis

dc:descriptionSince the 1920’s hypertension has been identified as an important indicator for the possible loss of kidney function. In this retrospective long-term study, the kidney function of patients with malignant hypertension and resulting secondary malignant neprhosklerosis was compared to the kidney function of patients with glomerulonephritis and secondary hypertonicity, focusing on the question of whether both groups of patients show the same prognosis in their kidney function. The study included 10 patients with nephrosklerosis (Group A) and 10 patients with glomerulonephritis (Group B) with an average history of 6.5 years (1.5-16.5 years). The study was performed within the groups, as well as between the groups with patients that had the same gender, age, blood pressure and initial creatinine values. From the long-term study it can be concluded that there is no uniform course of the kidney function in any of the groups. The Kidney function deteriorated in 15 patients (Group A 6, Group B 9). Nine patients had to be dialyzed. The remaining 5 patients had either the same (Group A 3, Group B 0) or improved kidney function (Group A 1, Group B 1). A positive correlation between the initial and end value of 1/creatinin could only be concluded for the glomerulonephritis patients and the patients with deteriorated kidney function(p=0.0084). A significant correlation between blood pressure and kidney function could not be found. A sufficient antihypertensive therapy was only established in 16 patients. All five patients with improved or stable kidney function where sufficiently treated for hypertension. The four patients with persisting hypertension had deteriorating kidney function. Besides the investigation of hypertension, the effect of proteinuria on the progression of the kidney function was investigated. Patients with lower proteinuria had improved kidney function. The comparison of the patient groups with primary and secondary hypertension shows that glomerulonephritis is an additional factor in the prognosis of the kidney function. The patients in group B developed more often and rapid terminal kidney failure. More studies with a larger array of cases are necessary to confirm these results.
