Stroke is a neurological deficit with acute onset based on a cerebrovascular disease (infarction or haemorrhage); principally every part of the brain can be involved. Even vascular damage to subcortical structures, i.e. basal ganglia, thalamus and white matter leads to neurological and neuropsychological deficits. Aim of this study is to characterize the neuropsychological deficits and their development after vascular damage to basal ganglia and/or thalamus. Special emphasis is aimed on visual attention and executive “frontal” functions. Included were patients with an acute circumscribed bleeding or infarction of the basal ganglia and/or thalamus. Cortical damage was excluded by magnet resonance tomography. The patients underwent a broad neuropsychological test battery. Standardized tests are used. Finally 11 patients with an acute localised subcortical vascular lesion were submitted to this study. The results from neuropsychological testing show that the deficits are not of general character but predominantly involve visual attention and executive “frontal” functions. Memory deficits are rare. Reaction to pure visual sensory stimulation is equally impaired after right- and left-sided lesions; deficits of spatial orienting and executive visual attention dominate after right-sided lesions. Reactive cognitive flexibility is impaired due to lesions of left-sided basal ganglia but not thalamus. The deficits are caused by disruption of cortico-striato-pallido-thalamo-cortical neuronal loops and/or by disconnection of parts within the anterior and posterior attention network Data from neuroimaging indicate that these deficits are due to lesions of the subcortical nuclei and not white matter. Neuropsychological performance did not improve significantly.
