Interreadervarianz bei der HRCT- und CXR-Befundung in einer Längsschnittstudie bei ehemals asbeststaubexponierten Personen

In preventive medical ckeckups of formerly asbestos-exposed persons the computer tomography (CT) is gaining an increasing influence besides the standard x-ray scans. A number of studies has been performed to investigate the interreader variance of two observers evaluating radiographic appearances of pneumoconiosis. The following study examined the interreader variance between two readers evaluating high-resolution CT (HRCT) scans and x-ray scans of formerly asbestos-exposed persons. Over a period of four years, annual HRCT and x-ray scans were performed at 636 persons of a high-risk population. Each scan was evaluated by two independent experts. The x-ray findings were recorded in the ILO 1980 classification forms, the HRCT findings in an evaluation sheet especially developped for this case -the predecessor of a meanwhile internationally used evaluation sheet (Hering KG et al., Radiologie 2004 44: 500-511). Ten pairs of readers were formed of a collective of seven experts. The variables of the evaluation sheets were categorized in a way that both methods (HRCT and x-ray) were roughly comparable. Afterwards kappa-statistics were used to determine the interreader variance. According to the author's knowledge this is the first study worldwide to compare kappa of CT- and x-ray scans concerning pleura and parenchym after a purposeful categorization of the evaluation sheets as well as examining the improvement. It could be shown that the interreader variance is smaller in evaluation of HRCT scans than in x-ray scans, in evaluation of pleural lesion than of parenchymal lesion and that an improvement could be recorded.
