Retrobulbäre Hämodynamik und funktionelle Parameter unter Normo- und Hyperkapnie mit Normoxämie bei gesunden Probanden

dc:descriptionPurpose. We investigated the vasodilatative effects of carbon dioxide to ocular hemodynamics. To avoid the vasoconstrictive effect of a hypercanic-hyperoxic gas mixture our investigation took place under normal air. We tried to establish a testing for vasoreactivity in healthy subjects by adding carbon dioxide (+15% to base line) to normal air. The induced hypercapnia could be used as a diagnostic criterion for glaucoma patients later. Methods. Thirty one healthy subjects (age 31 years, from 21 to 48, 16 male, 15 female) were studied under normo- and hypercapnic conditions. Color Doppler imaging was used to measure peak systolic and end-diastolic velocities (PSV and EDV) and resistance- and pulsatility-indices in the ophthalmic and central retinal arteries. We measured systemic hemodynamics, contrast sensitivity, intraocular pressure and calculated the perfusion pressure also. Results. Hypercapnia had insignificent influence to systemic hemodynamics. Contrast sensitivity increased in all 4 spatiel frequencies (3 cpd: +0,061; 6 cpd: +0,065; 12 cpd: +0,082; 18 cpd: +0,052). Intraocular pressure was reduced from 13,9 to 13,0 mmHg ((p=0,0008), perfusion pressure increased from 44,7 to 46,9 mmHg (p=0,0013). In the ophthalmic artery PSV increased from 35,2 to 39,6 cm/sec (p=0,0001) and EDV from 7,9 to 9,9 cm/sec (p=0,0002). In the central retinal artery PSV increased from 11,5 to 13,6 cm/sec (p=0,0001) and EDV from 3,4 to 4,1 cm/sec (p=0,0054). Resistance- and pulsatility-indices were all reduced in both vessels. Conclusions. We found an increase of contrast sensitivity and perfusion pressure under carbon dioxide and a decrease of intraocular pressure. Blood velocities increased in the ophthalmic and central retinal arteries. We defined vasoreactivity as an increase of EDV more than 22,7% in the ophthalmic artery and more than 15,8% in the central retinal artery compared to baseline. If our results could be confirmed in glaucoma patients, this could play an important role for the kind of therapy at a very early point of glaucoma disease.
