Histologische Untersuchung von Silikonimplantatkapseln nach Explantation : Korrelation mit klinischen Beschwerden

Background: This study was undertaken to investigate long-term histological changes in the environment of breast implants and their correlation with clinical complaints at the time of capsular contracture defined by baker-score, with indication for im- and explantation, implant site (submuscular or subglandular), implant duration, and surface properties (smooth or textures implant type).Methods: The collagenous capsules of 43 silicone breast implants from 43 patients (17 smooth and 26 textured implants) were evaluated histologically for capsular thickness, the presence of inflammation, the amount of silicon and calcification in the capsule and the presence of synovial-like metaplasia of the inner surface of the capsule with light microscopy and polarised light. All parameters were correlated with the Baker score.Results: A significantly higher degree of Baker score was found with increasing patient age (p=0,0181), implant duration (p<0,0001) and capsular thickness (p=0,0487). A trend towards greater capsular thickness was documented in patients who had a breast augmentation for cosmetic reasons. Synovial-like metaplasia was seen in 23 capsules (53,5%). The highest incidence was found in textured implants with a duration of implantation of less than 5 years.Conclusions: The implant duration showed to be the most predictive factor for developing a capsular contracture. We managed to show a correlation between the symptomatology of contracture (such as breast firmness) and histological findings (such as capsular thickness).
