Gibt es einen First Night Effect bei ambulanter Polysomnographie? : Eine Untersuchung an fünfundzwanzig schlafgesunden Probanden

dc:descriptionRationale Today in-hospital polysomnography is the gold standard in diagnosis of sleep disorders. Ambulatory polysomnography might be an interesting alternative to this, not only because it can be done in the subjects’ usual surroundings, but also because it offers a cost-reducing potential. It has become routine to do at least two in-hospital PSGs in order to avoid the so called first night Effect, which describes the fact that sleep is better in the second night as a result of an assimilation to sleep laboratory conditions. This study will investigate if there is such an effect in a group of twenty-five healthy subjects when polysomnography is carried out in their usual surroundings at home.Methods Twenty five healthy subjects aged 20-69, 10 female and 15 male, had ambulatory polysomnography on two consecutive nights using the vitaport 2 system. The acquired data were analyzed by two independent physicians experienced in sleep diagnosis using the criteria established by Rechtschaffen and Kales. A multitude of parameters (Total sleep time, sleep efficiency, sleep onset and REM Latency and sleep architecture) were calculated and compared using a standard T-Test for dependent trials.Results There were some differences between the first and the second night for single individuals, however the average values for all tested parameters for this group were not statistically different for the two nights, with the level for significance being set at p=0.05. To investigate an influence of age and sex on the first night effect subgroups were investigated, but no statistical difference could be reported for any of these groups.Discussion In this study there was no first night effect when polysomnography was carried out at the subjects home. This could mean that for a number of reasons it might be sufficient to do just one polysomnography if it is done ambulatory to get reliable data of their usual sleep. This opens new horizons for the diagnosis of sleep disorders not only with regard to patients compliance, but also concerning cost reduction.
