'High-Risk'-Säugling mit Leistenhernie : für welches Anästhesieverfahren sollte man sich entscheiden? ; Vergleich zwischen der Spinalanästhesie und der Intubationsnarkose : eine retrospektive Studie und Literaturübersicht

Ex-premature infants are subject to a particularly high risk after surgical interventions involving anaesthesia, due to a tendency to experience disturbance of respiratory regulation. This retrospective study was designed to compare primarily the effects of general and spinal anaesthesia on the length of anaesthesia and surgery time in high-risk infants undergoing herniorrhaphy. No exclusions were made for preexisting conditions. Elective unilateral and bilateral inguinal hernia repairs and emergent incarcerated hernias were both considered. Four hundred and nineteen prematurely born infants under 6 months of age underwent herniorrhaphy from January 1995 to December 2001. One hundred and sixty-seven received spinal anaesthesia with hyperbaric or isobaric bupivacaine 0,5% with epinephrine 1:200000 in a dose range of 1,5-5 mg. Attempted lumbar puncture failed in sixteen infants. Two hundred and fifty-two received general anaesthesia with isoflurane (0,4-1,0 Vol.%). There were statistically significant differences in body weight at the operation, preexisting diseases (respiratory distress syndrome, bronchopulmonary dysplasia, mechanical ventilation, systemic infection, congenital malformation, very low birth weight, history of apnoea after birth), duration of surgery and anaesthesia between the two groups (p<0,05). On the contrary there was no statistically significant difference in urgency of herniorrhaphy (elective or emergent incarcerated) between the two groups (p=0,24). Spinal anaesthesia is a safe and effective alternative to general anaesthesia in these high-risk infants. Postoperative respiratory monitoring is recommended following all anaesthetic techniques.
