Subjektive und objektive Schlafqualität : Vergleich von Insomniepatienten und Kontrollprobanden in ambulanten und stationären Polysomnographien

25 Insomniacs were studied polysomnographically for two nights both at home and at a sleeping laboratory. An age- and gender-matched controll group was examined only at home. I compared the sleep recordings and the subjects' estimates of their sleep quality in the morning after the recording. Additionally their habitual sleep characteristics were evaluated using the Pittsburgh Sleep Quality Index. In the morning after the recording all subjects were able to estimate sleep parameters, e.g. sleep efficiency, total sleep time, sleep latency, quite correctly. But the patients tended to underestimate their amount of sleep at home, although they slept even better than in the laboratory. The ambulant recordings aggravated the contrast between objective findings and the subjective assessment of the patients' sleep. Comparing the recorded sleep and the estimation of their habitual sleep the good sleeper controls are prone to overestimate their sleep quality. In contrast to them the patients with insomnia underestimated the amount of time they slept and overestimated the amount of time it took them to get to sleep in comparison with the recorded data. This finding can be interpreted as a sleep misperception of the insomniacs that affects mainly the long term sleep evaluation. A daily sleeping log leads to a more realistic assessment of a sleep disorder than a questionaire concerning the habitual sleeping habits.
