Prospektive Erfassung und Nachuntersuchung nicht operierter Refluxpatienten in der Chirurgie

Abstract

dc:description

The dissertation investigates the prospective acquisition and examination of patients suffering from gastroesophageal reflux disease at the surgery department at the university hospital of the RWTH Aachen. The goal is the retrospective evaluation of the recommendation of a conservative therapy of gastroesophageal reflux disease. Here unto the subjective contentment of the patients with their medical condition since the examination serves as a basis of valuation. The initial collective of patients is composed of 312 patients who underwent an examination between 09/01/1992 and 03/08/2000 because of reflux symptoms at the university hospital of the RWTH Aachen. Of this collective 83 patients either did not meet the criteria of a funoplicatio and a conservative therapy was recommended or the patients did not have a funoplicatio at the university hospital of the RWTH Aachen despite the recommendation to have one based on met criteria and indication for surgery. These patients have been considered in this study and received a questionnaire which 69 of them answered. In parallel to the survey the diagnosis is investigated. The recommentation is based on the indication for surgery of the surgical department of the university hospital of the RWTH Aachen derived from the examination results of gastroscopy, pH-metry, manometry and clinical symptoms. Seven of the 69 patients (10,1%) reported unexpected long-term results. Thereby an impaired medical condition since the physical examination of patients who received a recommendation for a conservative therapy is considered an unexpected long-term result. Likewise an absence of symptoms or content with the physical condition of patients with a recommendation for surgery is considered an unexpected long-term result. The retrospective evaluation revealed in 89,9% of the cases that the correct recommendation either for conservative therapy or sugery had been given. There is no single examination (gastroscopy, pH-metry, manometry) sufficient to establish the indication for surgery and subsequently the recommendation for conservative therapy or surgery. With several patients of the collective at least one of the examinations is without pathologial findings. The complementary diagnosis satisfactorily provides evidence of gastroesophageal reflux disease.
