Stellenwert der radikalen Lymphadenektomie in der chirurgischen Behandlung des nichtkleinzelligen Bronchialkarzinoms

The extent of lymphadenectomy as an integral part of curative surgery has been the subject of debate. In some countries, mediastinal lymphadenectomy is a religion, whereas in other countries it is totally ignored and not performed. No sufficient data are available to asses whether mediastinal lymphadenectomy has a positive effect on the overall long term survival in patients who have undergone surgery for their disease. Several authors advocate the necessary of mediastinal lymph node dissection. Other surgeons believe that extensive surgery will not consistently improve survival. Four hundred fifty eight patients who underwent from 1990 to 2002 curative surgery for non small cell lung carcinoma in our hospital were reviewed retrospectively. The study was performed to compare the survival benefit from extensive dissection technique with lymph node sampling. We could not demonstrate any benefits from mediastinal lymphadenectomy. Only patients with mediastinal metastasis did have not statistical significant improve survival rate after extensive surgery. We conclude that patients after systematic lymphadenectomy did not show significantly improved outcome as compared to patients with lymph node sampling only. The extent of lymphadenectomy in the mediastinum remains controversial and need other multicenter evaluations.
