Object of this study was to document the experiences and establish normal values for 24 hour ambulatory blood pressure monitoring (ABPM) in children less then 4 years of age. Not much data is available in this age group. The results are based on the evaluation of 92 ABPM-protocols, analysed separately for age, weight and height in five groups respectively. The average period of recording was 24,15 hours; a mean of 38 or 55% of the intended measurements were successful. Systolic, mean and diastolic blood pressure rise with age, weight and height; the heart rate decreases. While the rise is significant in the first three groups, there is no or almost no increase of blood pressure in the older/taller/heavier children. The same pattern is seen when calculating the daytime-, nighttime-, sleep- and awake-mean-values. Compared to conventional single office blood pressure readings the mean values of this study can be found in the upper range of the published data in children up to six month of age and higher than the conventional blood pressure in all older children. Looking at mean blood pressure values of each hour and the circadian pattern a significant drop can be found between 11 pm and 8 am. The difference becomes much more distinct with age: there is almost no difference in infants and an impressive nighttime drop in children age 1 to 4 years. Finally, percentiles for normal blood pressure values are presented for the first time in this age group. Individual ambulatory blood pressure values can now be compared with these results and interpreted appropriately.
