Vergleich von Positronen-Emissionstomographie mit (18F-)2-Fluor-2-Deoxy-D-Glucose und Computertomographie zum Staging maligner Lymhome

dc:descriptionCOMPARISON OF POSITRONE-EMISSION-TOMOGRAPY WITH (18F-)2-FLUOR-2- DEOXY-GLUCOSE AND X-RAY COMPUTED TOMOGRAPHY IN STAGING OF MALIGNANT LYMPHOMA Introduction: Malignant lymphoma are malignant neoplasia of the lymphatic tissue. They can be classified in Hodgkin´s Disease (HD) and non-Hodgkin´s lymphoma (NHL) by histological criteria; of NHL exist high grade and low grade variants. Therapy of malignant lymphoma depends on various factors like histology, staging and special prognostic factors depending on the entity of disease. Aim of the study Aim of the study was the comparison of two different methods of staging examinations: a morpholgic one (x-ray computed tomography, CT) and a functional examination (positrone-emission-tomography, PET) with 18F-Fluorodeoxy-glucose. Methods: CT and FDG-PET studies of 65 patients with histological confirmed malignant lymphoma as primary were evaluated retrospectively first by visual examination and afterwards by quantification. FDG-PET examinations were quantified for standardized-uptake-value (SUV), corrected for partial-volume-effect. CT-examinations were interpretated following usual radiological criteria. Results: 28 of 65 patients were examinated with a primary disease. In PET 97 pathological accumulations of FDG were found in lymphtic nodules (nodal, n = 90) and extranodal tissue (n = 7). All pathological findings in CT (CT+) showed an increased accumulation of FDG (PET+) and thus were confirmed as pathological in PET (PET+CT+). 39 patients with a relapse (after therapy) showed 93 pathological findings, 47 identified with both CT and PET, 23 only with PET and 23 only in CT. Furthermore, a significant correlation between lesion diameter measured with both methods could be found in nodal lesions. Conclusion: The evaluated data show, that 1. PET with 18F-FDG is equivalent or superior to CT in pretherapeutic staging 2. quantification of diameter in PET is valid in nodal lesions; partial-volume correction of can be performed without measuring diameter in CT 3. malignancy grade has a significant influence on SUV.
