Untersuchung zur Sepsis mit multiresistenten gram-negativen Stäbchen (SMGNS)

Blood stream infection (BSI) due to gram-negative rods (GNR) is one of the most serious infections in the hospital setting and antibiotic resistance among gram-negative rods has become an increasing medical problem. The first objective of this case-control-study was to evaluate the influence of BSI due to multiresistant gram-negative rods (MGNR) on costs, lenght of stay (LOS), empirical antibiotic therapy and outcome of infected patients. A second objective was to identify risk factors for the aquisition of BSI due to MGNR. MGNR were defined as those GNR resistant to one carbapeneme or/and one third generation cephalosporine or/and one carbapeneme. Sensibel GNR (SGNR) were defined as those GNR sensibel to all drugs of these groups. 78 cases and 100 controls have been identified in the study period (1996/01/01 to 2000/31/12) and were matched according to sex, age and gram-negative species isolated. BSI of cases and controls was primarily a nosocomial infection. BSI with MGNR was associated with an 11% higher mortality regardless of patients morbidity before onset of BSI and a significant higher LOS in ICU. Less than 25% of cases received an appropriate empirical antibiotic treatment. Risk factors identified were LOS in the ICU, previous antibiotic treatment and previous mechanical ventilation. The additional costs per case were 7,900 Euro and more than a quarter million Euro per year for the center studied. An effective interdisciplinary prevention program is necessary to reduce the incidence of BSI with MGNR.
