Patienten-kontrollierte postoperative Ernährung in der elektiven Kolonchirurgie

This is a prospective clinical observational trial with 40 consecutive patients, undergoing a colonic resection. The goal of this trial was to compare between traditional late-enteral or conventional feeding (CF) after the resolution of postoperative ileus and patient-controlled feeding (PF) including intake of clear liquid starting postoperative day one. For this, the patients were divided into two groups of 20 patients each. Results: The patients of the PF-group could be discharged 2 days earlier on average than the patients of the CF-group (13,5 SD 1,7 vs. 15,5 SD 4,06 days). This difference was statistically significant. The average weight-loss during hospital stay was 1,6% lower in the PF-group than in the CF-group (3,4% vs. 5%). There were no significant differences in the recurrence of bowel activity. The first flatus in the PF- group was after 2,85 SD 0,75 days and in the CF-group after 2,95 SD 1,1 days. There was also no significant difference in the day of the first bowel movement (4,3 SD 1,8 days vs. 4,15 SD 1,6 days). Nausea was found in 5 patients of the PF-group and in 7 patients of the CF-group. 4 Patients of the PF-group and 1 Patient of the CF- group were vomiting. There were significant differences regarding electrolytes. Fewer patients of the PF-group showed hyponatremia than patients of the CF-group (3 patients vs. 11 patients). Also there were fewer hypokalemias in patients of PF-group compared to patients of the CF-group (3 patients vs. 5 patients). There were similar findings on the subject of hypocalcemia (7 patients vs. 13 patients). The degree of pain was similar in both groups, with no significant difference. There were fewer patients with fever (>38,5°C) in the PF-group than in the CF-group (1 patient vs. 3 patients). Overall complications were found in 8 patients of the PF-group and in 7 patients of the CF-group. Conclusions: All patients of the PF-group tolerated early postoperative feeding well without higher rate of complications, particularly no case of anastomotic leakage. The patients profited from early feeding and the well-being associated with it. The Patients of the PF-group could also be discharged earlier, which reduces the costs. Early postoperative feeding does not lead to a higher rate of complications and can be considered as safe and effective.
