Klinische Bewertung von Kronen und Brücken aus Lithium-Disilikat-Glaskeramik

dc:descriptionThe purpose of this prospective study was to evaluate the clinical performance of a lithium-disilicate substructure glass-ceramic (IPS Empress 2, Ivoclar-Vivadent) for crowns and fixed partial dentures (FPDs). Patients with severe prafunction, periodontitis or poor oral hygiene were excluded from the study. From December 1997 to October 1999, a total of 184 Empress 2 restaurations (138 crowns and 46 FPDs) have been placed in 73 patients. Clinical treatment was performed by five trained clinicians following a fixed protocol. All substructures were veneered by the corresponding fluor-apatite glass-ceramic. For placement either a dual-curing resin cement (Syntac/Variolink II, Ivoclar-Vivadent) or a glass-ionomer cement (Ketac Cem Maxicap, 3M ESPE) was used. Up to date (April 2002) 128 crowns and 43 FPDs were evaluated. Adhesive placement was employed for 96 and conventional cementation for 75 restorations. The mean observation period was 36 months. Eight absolute failures were recorded in the FPD group as a fracture in the connector (7) or abutment area (1). All fractured FPDs were placed in a location, excluded by manufacturs guidelines. SEM analyses of the fractured connectors showed smaller dimensions than recommended. Relative failures occured mainly as minor and extended chippings of the veneering material (15). More than three-year clinical evaluation has demonstrated, that the clinical and technical guidelines for Empress 2 must be strictly followed to achieve acceptable clinical performance. Further research has to be undertaken to analyze factors causing chippings.
