This study examines the demographic and epidemiological data of burn injuries of children treated from 1987 till 1998 in the burn centre Aachen. In addition, the scars and the psychological adjustment in children after a severe burn injury (>=10% TBSA = total burn surface area) were investigated. Approximately 30-40% of all burn injuries happen to children. Burns are the second reason of accidental death in children under the age of 4 and the third for older children. Although the expertise of burn care has increased, one of the most important aspects in the treatment of burn injuries is the prevention of these accidents. Our epidemiological study provides necessary information for strategies to prevent childhood burn accidents by identifying the typical burn victim and the predisposing factors of burn injuries. By means of the Vancouver Scar Scale we examined the scars and the correlating predictors of scar formation after a severe pediatric burn injury. Older children at point of the injury, full thickness burn injuries and injuries in the near of articulations have a tendency to scars with worse dimension. The psychological adjustment of the children suffering a severe burn injury with >=10% TBSA was also examined by means of psychological questionnaires. The aim was to identify predictors influencing the psychological outcome according to burn related and psychosocial variables. At the beginning of the study we assumed that the children surviving a severe burn injury suffer of psychological problems. Fortunately, we found out that the children do not suffer from psychological disorders and that their life is worth living. Nevertheless, we picked out some children with posttraumatic disorders and a worse psychological adjustment. This emphasises the need for continuous follow-up examinations even years after a burn injury.
