Die Inanspruchnahme medizinischer Leistungen, Krankheitsattributionen und Selbstwirksamkeitserwartungen nach aortokoronarer Bypassoperation : eine Zusammenhangsanalyse

This study examines the relationship between psychosocial stress before coronary surgery and the amount of health care utilization in a sample of 115 patients during postoperative hospitalization. The aim of the study is to test the hypothesis that there is a correlation between a high psychosocial stress profile and the utilization of medical care (so called high utilizers). The sample consists of 80,9% men and 19,1% women aged between 34 and 78 years. Psychosocial impact is assessed by self efficacy and illness attributions (ASF, AFKA). Detailed somatic factors concerning severity of the illness, inpatient course and the utilization of medical care (medication, technical examinations, consultations) are assessed by means of a documentation system. High utilizers divided themselves significantly from low utilizers by their age, their higher attribution to partnership and their lower self efficacy concerning the body and health. There was no correlation between health care utilization and severity of the illness or comorbidity. In contrast to our expectations patients with a higher severity of the illness tended to have higher self efficacy expectations. High utilizers could be classified correctly in 81,8% using the variables attribution to partnership, age, clinical severity and attribution to addiction, while low utilizers were classified correctly in 95,5%. The data analysis showed that about 30% of the patients during the postoperative period utilize 100% of the consultations, 45% of the technical examinations, 42% of the inpatient time and 74% of the demand medication.
